This was kinda interesting. And with no provocation other than it could. When the shortcut for Email was clicked. Outlook opened what looked like two instances of itself. The only noticeable difference was the icon on the upper left corner. One was the usual Outlook one (the rectangle with the clock in front). The second had the icon for the Inbox (the envelope and folder). File>Exit would close both windows, and clicking the shortcut brought them both back!
Fortunately it seems the fix is just as easy as the problem is mysterious. Click the "x" on the upper right corner (I chose the window with the Inbox icon). Now clicking the shortcut for Email only launches the main Outlook window.

if you open two windows in outlook then close it using the file menu, two will open the next time. close the extra using the x button. also, if /recycle is not on the QL shortcut's command line, a new window will open each time it's clicked. This does not mean two copies are running - just that you have two windows open.

Update: The most likely cause of the two window opening was caused by having opened the second window and then file exiting without realizing the second window was still open. It doesn't help when its the end of the day and being tired and in a rush to gtfo and go home. Sometimes hard to pay attention to these little details.
